Approval record
An approval record is the stored decision: who decided, when, whether they were a member or a door, and whether the outcome was approve, reject or request changes. It is what you still have when the people have left the thread.
The engine opens the record when an item enters an approval stage. Exit through the approved transition is refused until that record exists. A later decision can be added; the earlier one stays in the append-only log.
Reminders point at the portal and do not re-send a credential. A door handed over with no email and no phone falls back to the studio, because only the studio can reach that client.

- No. A comment is a note. An approval record is a decision the engine will not let the work leave without.
- Can the record be deleted?
- Not from the application. The log is append-only.
- Does a client see the record?
- They see the decision they are asked to make, through the door. The full audit log is for the workspace.
